Requesting medical evidence

Guidance on how to request medical records

You may wish to try to obtain copies of your medical records, or those of a close family member, for your own information or to support a claim to the Infected Blood Support Schemes. If making a claim to an Infected Blood Support Scheme, you should always contact the relevant scheme first, to find out what evidence they require.

The Inquiry recognises that accessing your medical records is not always straightforward and has therefore provided guidance to help you in doing this. This guidance also covers how to access someone else's medical records (with their consent) and how to access the medical records of a deceased family member.
